The goal of the Partners in Prevention initiative is to provide training, funding, and technical assistance to 11 La Porte Country schools for the implementation of evidence-based prevention curriculum.

“This has to be our biggest initiative that we’ve launched to date because it affects so many people,” said Jeffrey Bernel, Board Chair of the Healthcare Foundation of La Porte. “It is a great program, built on a model with a proven track record, and we’re excited to see it work here in La Porte County over the next three years.”

Friday morning, representatives from each school gathered together with HFL staff and board members, and the Partners in Prevention technical assistance and training team, to celebrate the new program’s launch.

Keynote speaker Jim McClelland, appointed by Indiana Governor Eric Holcomb to serve as Executive Director for Drug Prevention, Treatment, and Enforcement for the State of Indiana, addressed those assembled for breakfast.

“I consider you pioneers,” McClelland told his audience. “You’re really leading the way in this area of prevention, and I commend you for that.”

Opioid addiction has become one of the fastest growing health threats in the United States. In 2017, Indiana was among the top ten states with the highest number of opioid prescriptions in the county.

“Ultimately the solution to a lot of these problems is to prevent them from starting in the first place,” McClelland said. “I think the Partners in Prevention initiative is wonderful. It’s a great example of what communities can do to use programs that have solid evidence of significant impact in reducing substance abuse among young people.”

The initiative was organized in response to recommendations identified in the La Porte County Opioid Study, commissioned by the HFL and conducted with help from the Center for Health Policy at the Indiana University Richard M. Fairbanks School of Public Health.

Thanks to HFL’s support, an anticipated 17,500 children and teenagers, accounting for almost 97 percent of the county’s total student population, will engage in a three-year program emphasizing positive relationship-building and social competency development.

“Our hope is that after three years, prevention becomes a sustainable part of school culture,” said Maria Fruth, President and CEO of the Healthcare Foundation of La Porte.

Initiative curriculum was built on the “Prevention Matters” model developed by the Richard M. Fairbanks Foundation. The evidence-based program emphasizes learning in key areas like decision-making, problem solving, and emotional self-regulation.

Resulting outcomes demonstrated not just a reduction in instances of substance abuse among students— they have also been shown to combat instances of bullying and violence, improve academic achievement, and encourage regular attendance habits.

Mark Francesconi, Superintendent of La Porte Community Schools, sees the initiative as a powerful tool for increasing the safety and wellbeing of the children and teenagers in his care.

“It’s great to have the resources of the Healthcare Foundation of La Porte to be able to implement a program like Partners in Prevention,” Francesconi said. “We’re really blessed as a community to have something like this.”  For more information about Partners in Prevention, the model it is based on, and other resources regarding prevention-focused education efforts, please visit hflaporte.org/partnersinprevention/.

He was nominated Employee of the Year for 2017 by his coworkers.

“What I enjoy most at Addison is spending time with the residents and hearing what their lives have been like,” he said.

Outside of working at Addison, Christopher is a non-denominational minster. He currently is a worship assistant, communion steward and  spirit director under Pastor Charles Sturtmeijer.

Christopher’s long- and short-term goals are to have more active church services at Addison Pointe. Addison hosts devotion every Monday.  Residents also have time where they pray for one another. Many times the residents don’t have a lot of people visiting them so having a prayer group gives hope and a unity of faith at Addison Pointe. Christopher also works with many of the employees when they might have struggles in their personal lives.

Chris has two children: Odin, 11 and Isabella, 16. He has always has felt blessed by the Lord. He feels that being a chaplain for the residents at Addison allows him to give back.

An autograph session with 3-time Stanley Cup Champion Chicago Blackhawks’ Patrick Sharp, music from 97.1 FM The Drive and tours of the new facility are all part of the event.

Community Stroke & Rehabilitation Center is the newest addition to the Community Healthcare System family. A specialty hospital with a focus on inpatient rehabilitation, it represents the first of its kind for the healthcare system. The hospital features a 40-bed inpatient rehabilitation unit on the third and fourth floors with dedicated activity and gym space for therapy.

The unit also houses an area that mimics an apartment for patients to practice the activities of daily living in a supervised setting.

“Studies show that for patients who are recovering from a stroke, neurological conditions or joint replacement, intensive inpatient rehabilitation treatment can result in better outcomes with a shorter length of stay,” said Craig Bolda, Administrator, Community Stroke & Rehabilitation Center. “The new Community Stroke & Rehabilitation Center is packed with advanced technology, new comforts for patients and families and new efficiencies that will enable us to provide enhanced quality care and best outcomes. In this environment, we’ve given every consideration to how we can better serve our patients, and how we can improve their lives and mobility.”

In addition to the inpatient rehabilitation unit, Community Stroke & Rehabilitation Center offers a wide scope of outpatient services. The first floor is home to an immediate care center, providing quick treatment for non-life threatening illnesses, bumps, and bruises. The first floor also is home to outpatient physical, occupational and speech therapy, diagnostic imaging services, a clinical laboratory and physician offices.

The facility’s second floor is home to physician offices along with a Women’s Diagnostic Center. The center is dedicated to breast imaging, offering same-day mammography results and additional related testing under the guidance of fellowship-trained and dedicated breast radiologists.

In addition to a variety of medical services providing quality care, the hospital’s landscaping features a healing garden to aid in recovery and promote reflection.

While on tour of the facility, the public will be able to view patient rooms and an activity gym in the rehabilitation center. The tour also includes the immediate care center, imaging and laboratory areas, outpatient therapy services, the scenic healing garden, and the visitor’s café. The tour concludes with an autograph session with 3-time Stanley Cup Champion Chicago Blackhawks’ Patrick Sharp (limited to one per visitor) between 11 am and 1 pm.

Community Stroke & Rehabilitation Center is located at 10215 Broadway in Crown Point.  The specialty hospital is part of Community Healthcare System that includes Community Hospital, Munster, St. Catherine Hospital, East Chicago, St. Mary Medical Center, Hobart, and Hartsfield Village continuing care retirement community, Munster.  For more information about Community Stroke & Rehabilitation Center Open House and other Community Healthcare System programs and services, visit COMHS.org.
