What is an Accountable Care Organization? Find out at an Upcoming Town Hall

rot2Health care transformation is under way at Franciscan Alliance. The Franciscan Alliance Accountable Care Organization (ACO) works with Fee-for-Service Medicare to provide patients the highest-quality medical care in a cost effective way. ACOs are made up of teams of health care providers, who work together to coordinate patient care across all types of care settings.

Area residents are invited to attend upcoming town hall events to learn more about the Franciscan Alliance ACO’s role in coordinating care for Medicare beneficiaries. Be sure to bring your questions and feedback – we want to hear from you.

To find out more about the benefits of participating in an ACO, contact the Franciscan Alliance ACO Medicare beneficiary call center at (219) 836-3310 Monday through Friday between 8 a.m. and 4 p.m. The Franciscan Alliance ACO cannot choose which patients qualify for the program. However, you are more likely to be included if your primary care provider is in the ACO, and you are enrolled in Medicare parts A and B.
