These programs are designed to help patients and their caregivers manage their symptoms at home and avoid frequent trips to the emergency room and re-hospitalizations. They give patients the ability to cope with their illness more effectively, increase their caregiver's confidence and improve the quality of living for the entire family.

BreatheEazy is designed to help hospice patients with serious, advanced lung conditions breathe easier at home, where they want to be. Many of these patients frequently visit the doctor's office, emergency room and hospital due to breathing difficulties, fatigue and other worsening symptoms. These can be prevented or managed with BreatheEazy.
 
Hospice patients with serious, advanced heart disease can also avoid repeated hospital visits and stay in their homes with the help of CHC's HeartWize program. Symptoms such as breathing difficulties and fatigue can be dealt with at home by managing fluid intake, tracking the patient's weight and watching their diet and sodium intake.

Regular visits from Center for Hospice Care nurses trained in these specialty programs provide for proper assessment and management, allowing symptoms to be controlled at home.
