IU Health Sports Medicine in conjunction with the La Porte High School Athletic Department will be offering sports physicals for the 2016-2017 school year on Monday, April 18th form 5:00 - 8:00 pm at La Porte High School.
All present 8th - 11th graders that anticipate participating in athletics next year at LPHS are strongly encouraged to take advantage of this opportunity. All athletes must have a physical on file in the athletic office in order to participate in open gym, summer contests or conditioning at La Porte High School this summer or next school year.
The cost of the physical is $20.00 and 2016-17 physical forms may be picked up in the athletic office after March 1st. The La Porte High School Athletic Code of Conduct (orange book) may also be picked up and completed at this time as well.
