Wound Care Nurse
The purpose of this position is the oversight of the facility’s Wound Program that includes implementing treatment orders, assessing resident treatment needs and taking appropriate follow-up action.
This position reports directly to the Director of Nursing.
Duties of the position include but are not limited to:
- Supervise the facility’s Wound Program, implement/assess treatment needs of the residents and provide routine skills and procedures. Continuous striving for zero in-house acquired wounds. Conduct resident rounds to assess the skin condition. Assess all pressure/non-pressure areas. Be able to handle emergencies as they come up.
Qualification/Requirements: Must possess specific educational and experience requirements such as:
- A high school diploma or GED.
- A graduate of an accredited school of nursing and possess a valid RN or LPN license in good standing according to State and Federal requirements.
- Understands the philosophy of and knows the principles, techniques and skills of wound care.
Certified Nursing Assistant – (CNA)
Assist residents with bathing, toileting, oral hygiene, skin care, grooming, nail care and dressing needs. Feed residents, provide water/snacks and transfer residents to the dining room as needed. Check on and provide catheter care. Assist residents with turning, positioning, exercising, transferring and ambulation. Answers call lights. Check on and assist restrained residents. Maintain resident rooms (i.e., making beds, changing bed linens, etc). Observe/report changes in resident’s appetite, skin condition or behaviors. Offer emotional support to resident and their families. Maintain resident confidentiality. Report and document resident and/or visitor incidents, accidents, complaints or allegations.
Take and monitor vital signs, intake/outputs, weights and meal consumptions. Follow documentation requirements for functional maintenance plans, 24-hour reports, CNA flow sheets, monthly intake logs, BM flow records, behavior management records, toileting plans, restraint sheets, shower lists, skin care audits, cleaning schedules, protective device usage and other records.
Required qualifications:
- High school diploma or GED, unless working under a student permit.
- Certified by the Indiana State Department of Health (ISDH) as a C.N.A. in good standing. CNAs transferring from another state may work for 120 days while awaiting their Indiana certification. Graduating CNA students, not yet certified, may also work for 120 days while awaiting their Indiana certification.
- Thorough understanding of the principles of safe effective nursing practices within the State of Indiana.
- Working knowledge on the use of personal computers, e-mail accounts and other computer software programs such as Word, Excel and ADP; as well as minor office machines such as faxes and copiers.
Nursing Assistant in Training
The primary purpose of this position is providing indirect daily care, safety and comfort of the residents while enrolled in CNA Classes to obtain Certification.
Duties of this position include but are not limited to:
- Offering residents compassion and patience while assisting them with day-to-day life tasks such as dressing, bathing and feeding
- Assisting with inpatient charting and instruction, documenting any care that is given and report significant observations to the nursing staff.
Required qualifications:
- A high school diploma or GED, unless working under a student permit.
Dietary Aide
The primary purpose of this position is to assist with the daily functions of the Dietary Department and to maintain a clean, safe work environment for all resident meals.
This position reports directly to the Dietary Manager.
Duties for this position include but are not limited to:
- Provide assistance with residents’ daily meals such as prepare/portion food items, follow recipes/menus, tray set-up, delivery of meals, put away stock, washing/storing cooking utensils, bussing of tables, cleaning of dining room and sweeping/mopping kitchen/dining areas.
Required qualifications:
- Have an understanding of the safe dietary procedures and the geriatric community.
Desired qualifications include:
- High school diploma or GED is preferred but not required.
- Some Dietary experience is preferred.
RN Unit Manager
We are looking for an exceptional candidate that can provide daily care, health needs, safety, and comfort to our residents by providing professional assessments, documentation of the resident’s health and managing an assigned nursing unit, which includes Charge Nurses, RNs, LPNs, CNAs, and QMAs.
Duties of this position include but are not limited to:
- Make daily and periodic resident visits for observation. Maintain safe staffing levels. Make rounds with physician. Prepare and administer medications. Implement restorative nursing measures. Supervise/provide care to residents with special needs. Provide emotional support to and maintain confidentiality of the residents/families. Develop, implement, maintain up-to-date, document progress, and recommend revisions to resident care plans/MDS. Maintain up-to-date resident medical records. Coordinate assignments for Charge Nurses, RNs, LPNs, CNs and QMAs.
Required qualifications:
- A graduate of an accredited school of nursing and possess a valid RN license in good standing according to State and Federal requirements.
- Basic computer knowledge and skills (i.e., the ability to access and use the internet, Word, E-mail, etc.).
Housekeeper
The primary purpose of this position is providing full range housekeeping services to maintain the grounds, furnishings, equipment and facility. This also includes maintaining equipment in a safe and efficient manner and ensuring that a successful housekeeping programs maintained at all times.
This position reports to the Housekeeping Supervisor.
Duties will include but are not limited to:
- Provide housekeeping services and maintain a clean work environment. Follow established housekeeping guidelines and quality assurance standards. Maintain cleaning schedules for assigned area. Coordinate routine/terminal isolation procedures with Nursing Services. Maintain equipment/supplies. Proper use, storage and disposal of housekeeping chemicals. Handle infectious waste and/or blood/body fluids. Assure work areas are clean and proper storage of equipment, tools, supplies, etc. Help in lifting heavy equipment, supplies, etc.
Required qualifications:
- Have a thorough understanding of the principles of safe effective housekeeping practices within the State of Indiana.
Desired qualifications:
- A high school diploma or GED is preferred, but not required.
